Police started pursuing the male driver after he was spotted running a red light at Baulkham Hills around 7.45pm (AEDT) on Friday.
The man allegedly sped off and hit an ambulance before driving on, police said.
The ambulance had to be towed away.
The man’s car then collided with two more cars before crossing onto the wrong side of the road and crashing head-on into a third car.
His car then came to a standstill and the driver, a 21-year-old man, was arrested.
No one was seriously injured in the incident.
