The 13-year-old boy was left with life-threatening injuries after the crash near Pine Creek, about 22 kilometres south-west of Bundaberg, while a 38-year-old female passenger died at the scene.
Three other people were hospitalised in the crash, including the 39-year-old man driving the car in which the woman died, and a 15-year-old female passenger.
A baby girl who was also in the car was uninjured.
All five were from the western Queensland town of Miles.
